Subacute Therapy for a Safe Return to Independence

We are a 50-bed subacute facility located within the walls of West Suburban Medical Center. Our location within an acute care facility allows access to professional nursing care 24 hours a day, seven days a week, and enables us to provide the finest care for individuals requiring continued inpatient rehabilitation services.

Our Mission

Our caring team on Subacute Rehabilitation at West Suburban Medical Center focuses on your safe return to independence. We offer tailored rehabilitation programs in an inpatient setting for a wide range of medical conditions.

Our Services

A Team Approach: Your dedicated team will include physiatry, nursing, physical therapy, occupational therapy, speech language pathology, recreational therapy, social worker, psychology, dietary, and chaplains. We even have Animal-Assisted Therapy!

Discharge Planning: Your first team conference will take place within a week of your admission and will continue weekly. Full discharge planning may include family education and training, recommendations for equipment, ongoing therapy, and information on community resources that are available to you.

Continuity of Care: Our team will remain in continuous communication with your primary physician. Your primary care physician in addition to your consulting physicians including cardiology, pulmonology, renal, and surgery will have the ability to continue to follow you on Subacute Rehabilitation.
